Please note that when submitting this form that we cannot guarantee a specific amount of time for a spot to become available. Our program fills vacant spots on an individualized basis and many factors contribute to whom will be selected for each spot. Thank you for your patience and understanding with this system.
Participant Application & Health History Form
This link will take you to an online Google Docs form that will be submitted directly to our program office. This is the form required for all current program participants to complete prior to their first lesson and as directed by program instructors periodically when files need updated. Please do not complete this form until you have been directed to do so.
Day Camp Registration Form (Not currently available)
This is a Google Doc that will be submitted directly to our program office. Please complete this form to enroll your child in any of our camps.
